Session Rates Without Insurance Coverage

• $135 for individual sessions

• $150 couples and family sessions

 

Insurance
Sessions may be covered in full or in part by your health insurance or employee benefit plan. I accept most private insurance and Employee Assistance Plans (EAP).  I am unable to accept Medicaid or Medicare plans. 

Review your health insurance coverage to determine answers to these questions:

• Do I have mental health insurance benefits?

• What is my deductible and has it been met?  What is my co-insurance percentage after my deductible is met?

• How many sessions per year does my health insurance cover?

• For TRICARE clients, is approval required from my primary care physician?

 

Forms and Letters

$25 for completed forms

$25 for completed letters

 

Payment
Cash, checks, debit/credit cards, and HSA cards are accepted for payment.

Cancellation Policy
If you are unable to attend your scheduled session, you must notify me a minimum of 24 hours in advance or you will be required to pay a $75 cancellation fee, unless it is an emergency or medical issue.
